help with getting a passport?

I'm looking for some help in getting a passport. When I was young my birth certificate, obviously had the name of my father. my mom and dad later separated and she remarried. never adopted me but somehow when I turned 16 my national insurance matters came through my step dads name.i so I had to change my name to a name affidavit or otherwise. I chose my parents step name. I did it simply through a court, just a piece of paper signed. i then used to obtain my driver's license. I have since lost this piece of paper. I never had any intention to travel abroad so I did not think I would need a passport. Now I have to think about getting one, as I follow my father who lives in Thailand and that at some point to go see him. will my birth certificate and driver's license without being sufficient the affidavit, if not I do. thanks

As above you need to read through the information on http://www.direct.gov.uk/en/TravelAndTransport/Passports/Applyingforyourfirstadultpassport/DG_174104 according to this you will need to have their paper made the name change, which if lost, you need to look, or repeat the process, or bring your passport to his birth name (that really complicated things for you when you need an identity document)
