First of all there is to know how to design a database, and you must know the proper accounting practice. Then you have to talk to someone clinic or hospital. Each area of medicine does things differently. (The patient database at a large hospital, usually not on the same computer as the purchase, AR, AP and GL database – but could be in a small clinic.) We are talking about a multi-year, multi-person project – not something that someone can give you an answer short for Yahoo.
