Hiring a rental car in Italy?

I have a driving license in the UK and USA a driver's license. Be legal to rent a car in Italy licensed either have to apply for (and get) International Driving Permit, but my question is Woud there any difference in the amount of insurance that we pay, if I use my American license in front of my UK license?

For Italy the car rental use your UK license is part of the EU and that definitely does not need an international driver's license. The only other conditions are that you are over 21 years of age.
